Enrollment FORMS

To enroll, the following forms will be collected before a child’s first day:

• Enrollment and Tuition Contract. Please contact Site Director for the correct contract based on your child’s schedule
• Registration fee & first week of child care tuition
• Registration Form (All Fields Completed)
• Health Care Summary (Required Within 30 Days of Enrollment)
• Immunization Records
• Permission Form
• Consent For Non-Prescription Medication
• ACH Authorization Form
• Key Fob Agreement – Parent Version
• Getting To Know Your Infant (Infants Only)
• Infant Dietary Plan (Infants Only)
• Infant Food Introduction (Infants Only)
• Getting To Know Your Toddler – Preschooler (Toddlers & Preschoolers Only)
• Dietary Plan (Toddlers & Preschoolers Only)
